Fiduciary Assessment and Recommendation Report
Licensed Fiduciary will conduct an independent assessment of the client to include – health status, functional ability, and daily assistence required. Fiduciary will then provide guardian specific recommendations and requests that would benefit the individual, including treatment needs. To include:
- First Client Visit with ZenGuardianship Fiduciary
- Purpose Driven Assessment (termination of guardianship, petition for new guardianship, change in authority)
- Medical Appointment requiring Fiduciary advocacy
- Attendance at Multidisciplinary Team Meetings
Routine Visits and Monitoring Services
Face to face visit with client to identify client preferences, ongoing or changing needs, and meet with social supports. Visits include interviews with facility staff and review of pertinent documentation to ensure welfare of the client.
Extraordinary Services
Services rendered on behalf of the client requiring immediate response. All extraordinary services will be approved by the legal guardian prior to additional services. This includes end of life planning, intervention required with medical provider, Adult Protective Services (APS) and law enforcement.
